Incident Investigation Survey

Survey Terms and Conditions

The incident investigation survey and gift card draw (Survey) is run on behalf of Incident Analytics, with its registered office in Victoria, Australia (Incident Analytics), and is open from 9:00am 18th January 2024 AEST to 11:59pm 4th of April 2024 AEST (the Survey Period). Incident Analytics may extend, cancel or amend this Survey and/or review and update these terms and conditions on its website at any time, at its sole discretion.

To be eligible to enter the Survey (Eligible Entrant), entrants must:

  1. Have accessed the incident investigation survey during the survey period; and

  2. be 18 years of age or older (or the age of majority in entrant’s region).

For the purposes of the Survey, an “Eligible Entrant” will be a safety professional. It excludes any person deemed a competitor to the Incident Analytics brand and/or safety consultants that contract to any competitors of the Incident Analytics brand. The classification of a competitor is at the discretion of the Incident Analytics team.

Incident Analytics employees and contractors are not eligible to enter the Survey.

In order to enter the Survey, Eligible Entrants must, by the close of the Survey Period:

  1. have validly completed each of the questions in the survey during the survey period; and

  2. have provided their contact details and validly entered their email address.

There is a limit of one (1) entry per Eligible Entrant.

Eligible Entrants will have a chance to win one of five prepaid gift cards (Prize) and the monetary value of the gift card will differ by each region and be the equivalent of $50AUD.

There are a total of five (5) Prizes available to be won and each gift card is subject to relevant terms and conditions of use including relevant expiry dates.

Five (5) winners (Winners) will be drawn from all correct entries following the end of the Survey Period on (Friday 5 April). The draw will take place in Queensland, Australia by Rose Street Digital.

The Winner/s will be notified by email, or using any other contact details you give us and by completing the survey, you also consent to Incident Analytics nominated agency, Rose Street Digital, contacting you if you are a Winner. If we have made a reasonable attempt to contact a Winner, and a Winner has not responded within three (3) business days, or a Winner is unable to use the Prize, the Prize will be forfeited and Incident Analytics will randomly choose another Winner. In the event of any dispute, Incident Analytics decision is final.

Incident Analytics does not warrant that the Prize will meet the Winners’ requirements or that it will be suitable for any particular purpose. To avoid doubt, all implied conditions or warranties are excluded in so far as is permitted by law, including (without limitation) warranties of merchantability and fitness for purpose.

The Prize cannot be transferred to others, or exchanged for cash or other benefits and cannot be split.

To the fullest extent permitted by law, Incident Analytics and its affiliated companies shall not be liable in any way (including, without limitation, in negligence), for any loss or damage whatsoever suffered (including, without limitation, direct, indirect or consequential loss), or for any loss or damage or personal injury suffered or sustained in connection with the Survey.

By participating in the Survey and by submitting any contact details, each Participant consents to receive the latest news, special offers, promotions and information from Incident Analytics by email, TXT or other channels from time to time. If at any stage you do not want to receive this information then please let us know, or use the ‘unsubscribe’ feature on communications we send you.

The Winner agrees that Incident Analytics may withdraw the Winner’s right to their Prize if:

  1. Incident Analytics name is, or is likely to be in Incident Analytics reasonable opinion, damaged or brought into disrepute or ridicule by the Winner or by being associated with the Winner; or

  2. The Winner is involved directly or indirectly in any situation (whether the relevant situation occurred before, during or after the Survey Period) which brings, or in the reasonable opinion of Incident Analytics is likely to bring, any of the Winner, Incident Analytics or the Survey into disrepute, contempt, scandal or ridicule.

Any personal information which we collect from you can be used in accordance with Incident Analytics Privacy Policy.
